Introduction: The aim of this review article was to analyze in details the mechanism drugs’ effects treatment and prevention a migraine attack, as well discuss hypotheses pathogenesis.
 Migraine attack agents: main agents for have an anti-nociceptive activity.
 Agents preventive treatment: β-blocker propranolol also has anti-serotonin analgesic activities, most drugs used prophylactic vasodilating Vascular hypothesis pathogenesis: Despite numerous studies that expanded our understanding pathogenesis, importance vascular component pathogenesis disease not questioned yet.
 Neurogenic cortical spreading depression: It is necessary take into account points context pathophysiology migraine.
 Neurochemical serotonin Serotonin plays important role Trigemino-vascular trigemino-vascular claims solve problem pain.
 ischemic brain damage: risk factor stroke cognitive disorders.
 Search new anti-ischemic anti-migraine preparations: A methodology search proposed.
 Conclusion: Belonging drug one or another pharmacological group does always correspond its therapeutic effect on pathogenetic processes migraine. with variety forms cannot fit only proposed disease.
